Insurance and Provider Information

Insurance Plans and Medical Providers typically expect you – the member – to know what it pays for and to choose doctors that are in the plan’s network.

  • Check Your Insurance Card or Member Portal

    Look at your insurance card for the carrier’s name and for the customer service phone number and the website.  Most insurance providers have online portals or a mobile app where you can log in and view your benefits.
    Tip: Be sure to keep track of login information!

 

  • Log In to your Insurance Provider’s Website

    (many provider website links can be found below)

Have your insurance card handy.  Use your Member ID or Username to log in. Look for the option for Benefits, Coverage, or Vision to see what services are covered:

    • eye examination – what is included or what is extra?
    • glasses or contact lenses – how often and what are the restrictions?
  • Search for In-Network Vision Providers

    Go to find a provider, by zip code, specialty, or location.  Out of pocket costs are typically less when using providers who are currently in network with your plan.

    Tip: Confirm that the eye doctor is in network both when you schedule AND when you arrive for your appointment.

  • Call Member Service

    Never hesitate to call the number on the back of your card if you are unsure what your plan covers.  They can explain your vision care benefits and confirm any copays or coverage limitations. Some insurance companies offer wellness tools, reminders for annual services or discounts.
    Tip: Be sure to ask if there are any other services such as TRANSPORTATION included.

 

  • If You Are Employed or Have a Case Manager – Ask for Assistance

    If you get insurance through your job, HR can help you understand your benefits, how to contact the insurance company, and may even help if there are issues with claims.  If you have insurance that includes support from a case manager, ask that person to help you explore vision coverage.

  • Get Organized!

    Establish a central location to keep track of insurance, medical record access, customer service conversations.  Create a list of reminders for annual examinations – including a comprehensive eye examination.
